Every therapist must keep and maintain clinical notes. Clinical notes must be kept for five to seven years for adults, and five to seven years after the client’s 18th birthday for minors (the time period is prescribed by your state’s laws.). In the event of a therapist’s incapacitation, death, or transfer, it is important to maintain clinical notes.
