When closing a therapy practice, there is much to be done. Therapists must ensure that client records remain accessible. Clients must be notified in writing prior to closure so that they may request records or transfer records to another practitioner. Clients are entitled to a copy of their record, but only with signed authorization. Never give a client (or anyone else) the original and only release copies with valid, signed authorizations.
