The Family Educational Rights and Privacy Act or FERPA is a Federal law protecting the privacy of student education records. According to the US Department of Education, this law applies to all schools receiving funds under an applicable program under this entity. Generally schools must have written permission from the parent in order to release any information from a student’s education record.

Appropriate school personnel, school counselor, and parent of client should sign this form in each other’s presence to ensure that there are no questions about the client’s intent to authorize disclosure of the information. Keep a copy of the form in the student/client’s permanent file, and provide a copy to the client’s parent, if requested.
